Adjusters / Nurse Case Managers

At Northern Indiana Hand & Wrist Center, we are committed to helping your patient return to work as quickly as possible.

To schedule an appointment for a workers’ compensation patient, please call 574-968-2832 and provide the following information:

• General patient information (name, address, phone, date of birth)
• Employer
• Workers’ Compensation Carrier
• Claim #
• Date of Injury
• Adjuster and/or NCM Telephone and Fax Numbers

To accommodate acute injuries, we will make same-day appointments when the doctor is in-office.

Nurse case managers are welcome to accompany the patient to his/her appointment.  Upon checking out, you and the patient will be provided with current work restrictions as well as an office visit summary.  Electronic office notes are generally faxed within 24 – 48 hours of the appointment.
